- Evaluating – We’ll begin an effective, caring evaluation process to help you determine the best solutions for your senior’s unique situation.
- Coordinating – We’ll even coordinate resources for you, make recommendations, provide education and family counseling to manage and adjust to needed change.
- Collaborating – We’ll coordinate with the senior’s doctor for health and medication issues.
- Implementing – We’ll identify and review any legal requirements including Advanced Directives and guardianship as needed.
- Planning – We’ll assist with financial planning and options to cover needed care.
Fees for services are generally covered by Medicare and major insurance plans with a physician’s approval. Private pay for services is also available.
